These were my plans for 2013.God who knows the future must have a laugh. He didnot gIve me chance for my cardiac checkup. I had heart attack in february. On investigation it was found I was having tripple vessel CAD. Rima Lima Y was done in Medica. I went in total depression. Gradually I recovered. In the process i lost 10 kg wt. Now i am having a lot of medicine. Food restrictions are on. I have crossed I crore. I shifted to my flat. I am doing laparoscopy in rampurhat more confidently. I am getting little more recognition from my colleauges.

In dec Aheli was suffering from respiratory distress. She was treated for asthma for 1 month. But she was not cured. CT thrax revealed foreignbody in rt main bronchus. She was admitted in VISION CARE hospital. Prof S Mitra did bronchoscopy and took it out piecemeal. she recovered. it costed 34 thousand though Prof Mitra didnot take any surgeon's fee.

I have started doing Pranayam and yoga. I am walking regularly. Yoga has helped to increase my mental peace and concentration.

IN SHORT 2013 WAS THE WORST YEAR I HAD SO FAR IN MY LIFE.

OH GOD! MAKE THE COMING YEARS TENSIONFREE ,PAINFREE,DISEASEFREE, AND JOYFUL.

27th and 9th lap chole

My last lap chole pt was from kaitha
Lean and thin.properly choosen. Entry was perfect and demonstrated to dr susovan Banerjee! Gall bladder was easy . Cystic duct was short. It took about one hour.. I was happy.

26th and 8th lap chole

After two months, I did another lap chole. Female patient of forty five. Entry.... Perfect
Gallbladder was green
Calot's triangle dissection was OK cystic duct and artery was nicely identified and ligated.
Liverbed dissection was perfect. It took about 40 mins.
Satisfactory.

Jo dear gia

Prof Biswas uses to say Jo dor via so mar gia . This holds good for surgery.
You have to be bold and daring to do surgery. Things may go wrong but you must gather all your knowledge and experience and tide over it

Why this is not angina

1. It is vague in nature.
2. It decreases on continued walking.
3. Sometimes it occurs at rest or during initial movement after long rest.
4. Similar pain occurs on taking deep breathing.
5. As far as I can remember its nauture is different from angina I used to have before Ami
6. It persisists for quite some time even after taking rest.
7. Pain does not increase on continued walking, it decreases on the contrary.
8. Pain occurs in different parts of chest wall.
9. More often it is more lateral.
10. It has decreased in intensity over time.
11. It does not radiate.

Riding the devil

I have handed over the papers of my disputed land to laltu. I know it is dangerous. But if I get back some of my hard earned money back, I shall be happy. Let me see if it succeeds.

Aheli crisis

Aheli crisis is not yet over. Aheli is having a little bit of resp distress. It has decreased but it has not disappeared. Long battle ahead.

In the morning I left for burdwan, I was in the train rinku informed that aheli is suddenly having resp distress. I asked her to visit Dr Chowdhuri. He gave some med. But disease worsened. I had very nervous moment. It waxened and waned. Lot of drugs were given. She was sometimes better and sometimes worse. Finally at about 1 pm she slept. She felt a little better. I felt little relieved. Rinku is a brave lady, she faced the tough situations alone.

Sachin retired from cricket today. He has been associated with so memories. Not attending or during housestaffship, stopping marketing to see sachan's innings. We love you sachin. He has been awarded bharatratna.

Shantanu aich

I hate this man. I have never hated a man. I was envious of many. I have sympathy for many people. I love many but I hate this man. He is a doctor. He fancys himself as a politician. He has contacts with all the persons we wanted to avoid. He never goes to hospital. I do not know how does he manage. He never goes out of the room. He locks the door both from outside and inside. He always thinks that people may murder him. He continuously telephones other people. He always lies to them. He will wear socks and monkey cap in hot weather. Nauseating people will come to meet him and talk for hours. I stay here once in 3 wks. But find this time terrible, and horrible. I hate this man

Joining hospital

After puja vacation I am going to join hospital after 11 days. It is really a long time since I got so much puja vacation. It was all by understanding. Today I couldn't board Varanasi express as it was fully loaded. I am going by local train. During the puja I couldn't do exercise and ate little more than usual. My blood sugar was normal but I gained a little wt.

Just to sign

I went to burdwan today just to sign. There is no ot. I went by 3134 dn ,signed, went to Medview to see a pt, rushed to station. Sealdah rampurhat express was standing in the platform 2. I checked whether any seat is available. There was plenty. So I boarded the train. I wanted to cancel my reservation in sahid express but I could not.

1 crore

Last year I set a target of making it to 1 crore. After by pass surgery I stopped updating my assets and bank statements. Now I have updated them and found my total investment crossed the target
It was a great feeling. My next target is 10 crore.
I don't know whether I can reach there with my compromised coronary artery.

Stairs

Today I ran up the stairs in station flyover without any difficulty and did not have any chest pain
I was relieved . we are going to Kolkata on Friday for my health checkup. Barda's health checkup is also a concern.

25th and 7th lap

Finally I resumed laparoscopy in Nh. I felt a lot more confident. Entry was perfect. Gallbladder was green. But I made the epigastric port a little lower, so I had difficulty in getting around the cystic duct. Lesson learnt from this that epigastric port should not be too low. It is better to err in the upper side. Cystic artery could not be identified separately. It was cauterized in the process. Liver bed dissection was better. Though gallbladder was punctured at one place. Gall bladder was removed intact.

24th lap chole

A lean and thin female patient. Entry was perfect. Gallbladder neck has little bit of adhesion. Cystic duct and artery was dissected out. Clip applicator was not functioning. It bled even after clipping twice. It was coagulated. Duct was clipped, and one extra corporeal knot was given. Liver bed dissection was as usual messy. Stones spilled which was taken out with desjardine forcefs.
Utpal's open cholecysyectomy was uneventful. He is complaing of lot of pain. Paracetamol infusion was given.

Utpal's operation

Today Utpal will be operated for gallstones. I preferred the open approach as instruments in medical college ( for laparoscopy) is not very good. Prof R S Biswas is going to operate. Everybody is very tense. who knows the future? I t is likely to be uneventful.
Barda is having chest pain. TMT is positive. Angiography needs to be done. Tension is around him also. All expect me to arrange everything for him. But nobody is caring that I am not in the best of physical condition. Sometimes I am having reeling of head. Chest pain of various kind is there. I hope all are insignificant.

Maa Durga

Durga puja is around. Last year everything was nicely poised. I was waiting for it. But an event has changed my life. I want to be happy but I cannot become fully happy. My father rings everyday to hear me. My mother also rings often. I am not happy. Every time It plays in the subconscious level that I had a bypass surgery. ... I have heart disease. Things may end suddenly. When I look around , so many old persons enjoying their life. I am unfortunate. Then I think, so many people die at tender age. Many die of AMI , I survived and reached bypass table. With diet restriction I can live normal span of life. Sometime diet restrictions make me feel depressed. But there are many who ate vegetarian by choice. There are many who cannot afford two square meals. I am. More fortunate than them.

25 to 30 years only.

I read today in a magazine, Dr Satyajit Bose has written that Lima rima y can last for 25 to 30 years. So no other intervention required in my life. Let me see how long shall I be able to survive. Will I be writing the blog at the age of seventy. I do not know. Let me see whether we healthy lifestyle and yoga takes me there.

MES, was it fruitful?

I joined mes three years ago, I was little tensed at that time about the future. Whether it was a good decision or bad, I was not sure. Let me review.
1. If I get transferred to Calcutta, I will be in a soup. I have to buy a flat to combat the situation.
2. No tension at work.
3. I am doing a lot of complicated operations at medical college.
4. It has a positive effect on my confidence.,and private practice at rph
5. I have lost about 10 kilos of wt and bit of myocardium.

Appreciation

One year ago when there was unit reshuffle, no unit wanted me. I was terribly hurt. This thing never happened to me. After 1 year I am in demand. Prof R S Biswas said he could not spare me. Every one from my unit protested my shifting to other unit. Sandip said I can stay the way I like if I go to his unit. In one year everybody started to like me, love me, appreciate me. This is a tremendous achievement. Prof Biswas gifted me one hilsa fish he bought from sagar. Today we performed one hiatus hernia operation. That's great. I think joining MES is satisfying.

23rd lap chole

My 23rd lap chole was a young female with a single stone. Entry was perfect. Little bit of adhesion here and there. Calot's triangle was nicely dissected. Then I had little problem in dissecting GB from liverbed. GB was punctured at 1 place. But everything was nice at the end.

Things to do

I have a few things to do. I am not able to do it for reason of laziness and unknown fear. I must do them.
They are.....
1.. boundary wall to my land.
2.. Close few bank account.
3.. ATM
4.. close my demat account
I want to give myself 1 month time to fix it. If I can't do this I can only blame myself.

choledochal cyst

My aim of joining MES 3 years ago was to do big operations, Today I removed a type 1 choledochal cyst and did a  ROUX EN Y hepaticojejunostomy, It was nicely done. I am Happy.

My 22nd lap was a young female, lean and thin. entry was perfect, gall bladder was jucy. iwas doing lap after 4month. cystic artery was dissected and clipped, but it slipped somehow. it was caught and cauterised..Prof R S Biswas was of opinion that cystic artery need not be clipped, it can be cauterised safely.
cystic duct was clipped and divided, then i took the hook and tried to lift from the liverbed. gallbladder was perforated twice. it was a little messy in the end. i must be careful regarding removal from liver 

Slight recovery

There is slight recovery.  But still I am suffering from financial crunch. With increase in no patient and operation there is a hint of recovery. I can not put reins on expenses. I must do something. What can I do. I must find  way out.

21st lap chole

It was a hefty male. And Gall bladder was difficult. Dense adhesion all around. Cystic duct was identified and clipped. Rest was a total mess. Only ant wall of gb was removed. Stone spilled.
Lesson learnt.... One should try to develop the plane between liver and gall bladder with Maryland . Too early use of hook may result in loss of plane.

Finamcial crunch

Every year I suffer from financial crisis during the month of Jan. As I go to conference every year during the month of Dec. So no income for about 15 days. Expenses double up during the tour. On return the dull practice in winter session make things worse, this year expenses related to flat has made things more difficult. About 25 thousand rupees are spent in mutations of land. All is making my life miserable. Hope to tide over the situation soon. A possible loss of rs 5lakh in a land has made it even worse.

Anxiety

The troubled times seem not be over. I really cannot tolerate this. The land problem is not over. I have lost about 50 thosand in share market. My money matters are little easy ward. Things are not smooth . I have to work out a steady plan for it.

Tense moments

goa

on 5.1.13 we boarded the Amarabati express at 11.30 pm at howrah station.  We never get good seats', so no point brooding over that. later on discovered that it has no pantry car. That caused a lot of problem. During the journey i met a foreigner who came from france.  In the later part train was moving through few tunnels. it was a new experience for me. tunnels  started after a station named Rock Castle. We saw a water fall named Dudh Sagar.

Hotel Nagoa Grande was a 3 star hotel. It has all the facilities like swimming pool. food is relatively cheap. Zuventus has  higher limit 2500 for daily expenses. we shall finish well below it. Its room service and house keeping is bit sloppy. I gave them a good thrashing, after that things were easy.

On 7th we visited calangute beach. It was a huge beach with lot of crowd.

Bagatora beach  It is a huge beach again with lot of people taking bath.

One should not board this train. Simply because there is no pantry car. They took ouq order. For lunch and at 12 noon they informed that they cannot arrange it.

And we have to buy it at bijoynagar station. That was totalla horrible.

<p>&lt;p&gt;&amp;lt;p&amp;gt;Yesterday we boarded Amarabati express at 11.30 p.m. Aheli was running in howrah station. She is very difficult to control . She developed lot of adayes.
I slept on the train. In the morning I discovered that there is no pantry car. So teas and coffee were late in coming. Rik was as usual impatient. I have informed my. Mother in the morning. She went to Lalbagh with Amiya yesterday for a picnic.
