Lookback 2014

Another year gone by.. When I look back at last year's promises i made are few done.
1. I have pushed my score by 30 l
2. Could not buy a flat or put boundary around my land.
3. Maintained my numbers.
4. Could not become vegiterian , on the contrary I attended conference at DEC and ate a lot of fish and chicken.
5. Did a few lap not as much as i wanted to.
6. I had A very good practice during this year, also got recognition from many quarters.
7.  I occasionally have chest pain, I really get confused at times.
8. I did health insurance.
9. Could boost Rik's education as much as I would love it.
10. Could not make term insurance.

Last day at hyderabad

We finished the conference at about 12 noon. Lunch was most unimpressive. We went to snow world. That was most fascinating thing I saw in Hyderabad. The place was full of snow  artificial snow fall was also there.  We were supposed to stay there for 1 hr, but we could stay there for only half an hour. Our feet and hands were aching in  intense cold. (Below 0 degree)
We visited Birla temple. It was at a higher area. Panoramic view of city could be seen from there.

Golkonda fort

We went to golkonda fort. It is a big fort on a small hill. You need to climb 367 steps to reach it's top. I could actually reach there. They say that if you clap at the bottom of the fort it will be heard at the top. But it cannot be verified . lot of people just claps for fun. Big walls, stones are there to see. It has a long history, there is a light and sound show in the evening. Amitabh bachhan's voice describes the history. In the evening we went to hitex hall 2 for banquet.

Lumbini park

After 2 pm we went to lumbini park which is basically a children's park by the side of Hussain sugar. It contained slips,cars, merry go round.  There is facility for boating , parasailing  in Hussain sugar which is basically a big lake. There is lot of traffic jam in hitec city.

Surgery workshop

Not the best workshop.2nd half showing only the endoscopy procedure. 1st half showed few open and lap procedure. Fund us first gall bladder was something I learnt..
In the evening we went to Karachi bakery and bought few biscuites cakes etc.

Ramoji film city

Today we went to see the ramoji film city. Tickets are priced at 800/per adult, which is quite high. They take by bus .  Bus service is quite good.  There are lot of buses ,one does not have to wait long. We saw many things for all day but could find nothing interesting. Even the stunt show was not very interesting. In movie magic they showed chroma effect,which I knew for years. So a full day was lost. It consists of huge area. There are gardens, different sets for shooting birds enclosure, butterfly enclosure. There was a dance show, circus show. But we have seen these things before in other place, so why should come a long distance and pay huge  money  for these stuff.

Hotel Trident

It is a five star hotel, room rent is 14000/day. Service is very good. All the room amenities like thermostat, 42 inch flat TV, electric kettle, locker etc!
Bathroom as always is amazing. What disturbs me about this hotel ( basically all five star hotel) is it's cost of food. Lunch or dinner for 3 person cost about five thousand rupees, a tea pot costs 500 rs. Three plates of noodles and three ice cream cost three thousand., special dinner like Christmas dinner cost 2500/ person. That is horrible.

Hyderabad day 2

Sudha car museum,..concept is funny. But actually it does not look funny. Cars in different shape like ball ,bat, lipstick camera looks funny, but we could not see them moving. So it looked boring. 
Chowmohalla  palace.... Huge palace once inhabited by Nijams. Takes a lot of time to see.
Charminar... Its beauty is in it's history. Laad bazaar by it's side showing lot of bangles. Pearls shop is also there.lot of bargaining skill is reqd to purchasing here.

Falaknuma express

We boarded falaknuma express at 7.25 am in Howrah station at PF no 20. It started in time. I always felt that if we have 4 family member then we can occupy one side of Ac two tier coach. So I bought Aheli's ticket, though she was just 3 years old. And my dream was fulfilled. Like as I used to do I took tea bought breakfast. But I could not enjoy it that much as there was a tension in my mind because of the disease.
I am happy that I am going to conference after two years.

Hyderabad

After a long time I am out with my family for a tour. We are going to Hyderabad. It is common capital of Telengana and AP. Asicon will be held here from 26 to 30. Now we are in biswabharati, going to Calcutta. Tomorrow we Wii board Falaknuma express from Howrah station.As I have gathered, it is known as city of pearls. Lot of jeweleries of pearls are available. Other places to visit are
1. Hussain sugar
2..Golconda fort
3'. Ramoji film city
4. Few museum ,temples, mosques

Nabanna at kaitha

40th lap chole

It was a medium built female. But  the gall bladder was thick walled- empyema. Omental adhesion at fundus. Initially I thought of converting.. Then I continued. It took 1 and half an hr but I  managed it. Entry was easy. Liver bed dissection was tedious but well done. It was level 6/7 GB

39th lapchole

Medium built female. Entry OK. Dissection  of calot's triangle okay. Liver bed was alright. Stone spilled during taking out of GB. So final part was little messy. Took 1 hr.

Decreasing pt.

During this part of time no of pt decreases. I was having around 30 pts everyday. It has now dropped to about 20.

38th and 15th lap chole

It was a medium built pt. Entry was horrible. Then it was okay. It was a level 1 GB. Nicely done in the end.

Bhai phota 2014

37th and 14th lap chole

It was a thinly built pt. Nicely done. Took about an hour. Little difficulty in entry. Quickly enlarged incision and got inside.

Last professional month

Last month was awesome so far was practice is concerned.
I performed about 110 operations, out of which 21 was gallbladder. Though I do not have proper stat, but I think this is highest in my life.
Only thing that I have done only one lap chole. I have to increase no of lap chole.

36th lap chole and 13th

Medium built female. Entry difficult. Gall bladder looked easy. Dissection produced confusion. It took 1 an half hour. Ultimately it was done. Not neatly, but somehow done.

Joining mes

Yesterday I performed 1 renal stone, 6 GB ,1appendix, and 2 piles and 1 dermoid operations. I cannot think that I would have been able to this much operation if I didn't join mes. It has made me very confident. Doing 10 plus OT in a day has become a routine than exception.

35th lap chole

Everything was okay. Anatomy was little confusing. Liver dissection was clumsy. But in the long run everything was well managed.

Lost my watch

For the 2nd time I lost my wrist watch. My wrist band was loose for some days and I was neglecting it. The result that it dropped from my wrist during my word round. I could not find it moral of the story... Do not neglect if your wrist band is loose.

Health check up

Going to Kolkata for a health check up. puja market will he done also. whenever I go for a checkup I get scared that doctor may find something wrong. I am tired of my disease. Can't it happen that I live without thinking about my disease?

Birthday

Yesterday was my birthday. My mother came to rampurhat. I am 46 now. How far can I go? It is terrible to live with a disease. Lucky are those who do not have a disease. Rik is 13 and Aheli is only 3. Miles to go.......

34th lap chole

It was level 1 GB. Entry was troublesome because of faulty trochar because gas was not going inside. So lession is when no gas is going one has to check the patency of trochar. Rest of the operation was a cake walk.

3134 dn

This train is now extended up to Delhi. It has few more extra coaches, including 1 a/c coach. But it is still neglected in the stations. Local and other trains go past it.
My problem in BMCH still exists.

Good practice

Now I am having a reasonably good practice. I am doing a lot of operation, and having 30 to 40 patients everyday. Patients are waiting to be operated. That's great. Literally I am not getting time to eat. Yesterday I operated on about 12 patients. Yes it is very tiring, but satisfying at the same time. I am having a little bit of problem with the hospital attendance, but I think I can get over it. Physically, I am feeling okay. Sometimes I feel pain in the back. But that too occurs even at rest and generally in the afternoon after fays hard work. So it is likely to be a muscle pain. I hope so.

Feeling sleepy

After a hard days work I always feel sleepy in the evening and cannot teach rik. I must find a way out of it.

C N banerjee

This frustrated gentleman is creating a hell lot of problem. This frustrated chap without much practice is after me. It is best to keep cool and the situation to settle. This gentleman does not like to go home on holidays and wants to be busy in leg pulling. I have seen this kind of people before. I think we should remain silent and for situation to settle. I am also thinking of changing my unit.

Whatever......

I travelled by train thrice a WK . I see a lot of people enjoying different food. The eat kachori, parota, sweets, spicy chicken, boilled egg, and what not. Lucky chaps. What I can eat is nuts ( that to with fear) matarbhaja. Matar bhaja is causing lot of flatulence.
What should I eat? Roti tarkari. I really do not know whether this kind of life style will lengthen my life. My retirement age has extended to 65. Can I live that long?

My surgery practice has increased considerably. I am earning a lot of money.Neill I be able to live up to ripe old age to enjoy the fruits of my labour

33rd lap chole

It was a lean and thin pt. Level 1 GB. All the structures were visible.
cystic duct isolated and clipped. I had again problem with artery. Liver bed was cleanly dissected. No bleeding.
Dr C N Banerjee was suddenly causing problem.

32nd & 12th lap chole

Again it was a relative of Dr P R Datta. Moderately built pt. Level 1 GB. Everything was okay but cystic artery was torn and coagulated. I must do something about cystic artery. Rest was uneventful. It was a high profile pt. Wife of a rice mill owner.

Total penectomy

This is another operation which I always wanted to do but I didn't get chance. Today I performed one in BMCH. This also justifies my joining MES.

Mes review

About 3 years ago, I took a decision to join mes. I was not very sure at that time whether it was a correct decision . let me review.
First the bad...
1. Chance of bad posting
2. Chance of bad unit posting.
3. regular travelling.
Now the good...
1. I have become a confident surgeon.
2. I am doing lap confidently
3. Even VIP pts are coming to me.
4. Practice is up.
5. No work related tension.
6. Theoretically I am also learning a lot.
7. I am happy.

31st and 11th lap chole

It was a distant relative of Dr p R Datta. It was level 4/5 gall bladder. Gallbladder could not be seen at first. Dense adhesion all around. they were teased off. calot's triangle dissected. Duct identified and clipped. Aretery coagulated. Liver bed dissection was gentle. Mild oozing here and there. Gallbladder was removed without any spillage.

Reeling of head

Whenever I talk about wt loss or health I have reeling of head. I can walk for about 40 mins without any chest pain, so things must be okay. Oh! God' can't I become normal again. pls help.

30th lap chole

I am back in my old unit. I did a lap chole on the first day. It was a hefty male. There was adhesion all around. I teesed off all the adhesion. Did a good job by dissecting the calot's. Duct and artery dissected and clipped. Liver bed dissection was done. But unfortunately bleeding started after that. After all the effort we could not find the source.we waited, put gall bladder in liver bed, tried to cauterize live bed. At the end bleeding was acceptable. Bwe put a wide bore drain and came out. Post operatively no blood was coming in the drain, pt was stable.
Less ion: one should have a good look at liver bed and stumps before removing last attachment of GB from liver. Because after removing GB from liver bed it is difficult to see the calot's.

Fast surgeon

Last Friday there was a huge hypothyroid patient having GB cal. Dr k Mondal was scared to do operation of that patient. I did the cholecystectomy in 20 minutes time. Dr Mondal was repeatedly telling that I was a fast surgeon. I liked it.

29th and 10th lap chole

Moderately built pt. Entry was perfect. Flimsy adhesion all around GB. Level 2 GB. Operation was done with authority and confidence. Liver bed dissection was okay.

Terribly upset

I am terribly. Upset at the unit change. There was smooth going in unit 3A along with Prof R S Biswas and Ashim. Dr Koushik Mondal isa bad person. Nobody likes to work with him. I shall never forgive Dr Naren Mukherjee for this.

New unit

Today I am posted in Monday unit. I hated this unit. But there are many thing which is not under my control. This is said to be temporary.
Dr tamanash chowdhury is on leave. So I have been posted there as replacement. There is scope of work in this unit as RMO is novice. I must utilise whatever chance I get.

Aheli's birthday

Aheli's birthday is never celebrated as rik's exam is going on. I bought a teddy for her.

28th lap chole

Middle aged woman, lean and thin. Entry was not perfect but managed. There was adhesion all around gb. It was teased off with confidence. Cystic artery and duct was dissected with confidence. Whole procedure took about 40 mins. Good going bro.
Beauty of this ot was that it was done in utter confidence and style.

Marriage anniversary

Yesterday was 26th Jan. Our 14th marriage anniversary. I remembered it. Rinku forgot. Most unimpressive way. She was sick and down in bed. I was just busy in practice. Next year I shall celebrate it.

Mollarpur

Yesterday I went mollarpur as opened a new chamber there there were 20 pets there. Not bad as first outing. It is worthwhile if I get pts. There were few surgical pts also. Let me see how it comes in the end.

healthy food habit

sometimes i eat rich food and get frightened. so i have decided to follow a rigid food calender.
1. i won't tave fish more than twice a wk.
2. eggwhite can be taken.
3 chicken not more than once a month.
4.plenty fruits and vegetables.
5. One cup of tea with sugar/day
6. Max 2 sweets/wk
7.no skip meal
8.nut once in wk
9.no junks.
