1. practice was low in january.
2. organised a family picnic with four of us.
3.bought a 60 gm gold necklace for rinku as marriage anniversery gift.
4. arranged saraswati puja in our rampurhat flat.
5.did few lap chole not as much as i would have liked to.
6. i had pain in the rt knee joint for which i had to do thorough investigation. an hematoma subperiosteal was found.
7. few operation had complications causing tensions.
8. vauge chest pain contiued throughout the year.
9 Aheli was admitted in st paul school
10. rik got 89 percent in board exam. admitted in narayana kolkata.  he stayed in hostel first failed to adjust then shifted to my flat.
11. i did my blood reports which were very encouraging .
12. i manged to decrease my body wt to 66.
13. actually i stopped taking sweets in later part of the year. but in last week of dec i took a lot.
14. my mother was operated for cataract in suri. she couldnot see after that. so i have to get her reoperated by moumita in kolkata.
15. i started going to kolkata every wk so lot of my time were spent  in the train.
16. after 13 yrs i went to islampur on a summon. met b. bera and others.
17. we went to jaisalmer and jaipur to attend asicon. enjoyed camel ride ,desert camp etc. i had no restriction in intake of food dor last 10 days. now roday 31st, i am retirning home and wish to tighten the noose.
18. Today is one of many yearending when i am in the train.

amer, city palace, jalmahal, howa mahohal

revisited those fortes after 13 yrs. least impressed to say the least. finally went to bapu bazar near howa mohal. it was crowded local market. good for shopping. Time to say good bye to jaipur. now another bad news our howrah jodhpur express is late by 5 hrs.
this is the worst train so far as the punctuality is concerned.

Rajasthan 26.12.2017

Yesterday we left Jaisalmer at 6am, and arrived Jaipur at 7 30pm. It was such along journey by car. Hotel Marriott is active star hotel where you have hardly anything to complain one just gets overawed.
The buffet dinner at night contained innumerable items and it was difficult to find which item we should eat. The bill nearly gave me a heart attack. It was  rs 7432 .
Today we went to workshop at 9 am. It was good but showing mostly the advanced lap surgery.

In workshops generally families are not allowed. But later on I learnt that they are allowed so I sent the driver to take them.
So much of items were there that we were finding it difficult to decide what to eat and what not.

Finally we  I came back at 5 30 pm.

Rajasthan 24.12.2017

we checked out from hotel at 10 am and went to golden forte. A guide went after us and we reluctantly hired him. Later on we realised it is impossible to tour golden temple without a guide. He was a perfect gentleman ,guided us properly told history showed all the parts clicked some beautiful picture from best spots. Then we  took us to a stone shop own by Bengali. He came here afyer seeing the  cinema সোনার বাংলা, then opened a shop here and never returned. people call him মুকুল who never returned back. we purchased few things from his shop and clicked a selfie with him.
then we took lunch in a resturant (chandan shree) outside fort.
After that we saw mandir palace. it was not impessive, so we left for Sam dunes.
Royal desert camp was impessive. The camel ride was totally entertaining. we talked to বাবা over video call and showed the desert. Aheli was totally thrilled though she was a little worried about the camels.
on return from dunes we were offered snacks and tea followed by local cultural programme which totally enjoyable. After that dinner was also impessive. Thoug we didnot try ডাল বাটি চুরমা this time.  but  i took "gatte ki sabji".  But i didnot like it. After sunset temperature was going down. with wind blowing it was shivering cold. we entered our desert camp  and tried to fall asleep as our plan is to set out for jaipur early morning.

rajasthan 23 12.2017

we had breakfast in the morning. then left for Tanot Mata ki Temple. It was 120 kms away. There was lot of wind mill on the way. roads are clean but but roller coaster type, moving up and down.
tanot mata temple is maintained by BSF.
It was famous because during war of 1965, about 3000 bombs fired at this temple and none exploded. I prayed for my good health and rik's success in exam.
Then we went to longewalla.It is the place where indo pak war took place in 1971, and our braveheart made the enemy bite the dust. we warched a mini cineme about the war also.
Then we went to BadaBagh. it contains cenotaphs of various royals of rajasthan.
good for clicking photos etc.
then we tried to vist selim sing ki havelli. we saw it from outside, it was not impressive.
Then we went to Gadsiar lake, it was  big lake with  boating facility and nothing else.

rajasthan 22.12.2017

we got down from train at  2.15 pm. Found the driver.  we found the janata sweets.bought "piyaj ki kachuri" and samosa. Then went to priya resturant for lunch.
after that it was five hour journey to jaisalmir.
google map helped a lot.
we found "chouki dhani" a four star hotel. Rooms are big. but two rooms are not side by side. i had to shout a bit before a proper arrangements were done. we dined at night. Buffet was in the package. So it was ordinary  at best.
Rik was given single seperate room. He seemed happy.

Train is running late by 7 hrs. This is worst train I have ever boarded. Only Tanot Mata can save me. Otherwise all my plan programmes will be in soup.

so far punctuality is considered this train is worst. it always runs late by 3 hrs. today it is late by 5 hrs. train stuff told me thain actually reaches jodhpur everyday. otherwise i thought it may not  reach. it is so slow and stops more , moves less. food, other things are avg .
i drank "baghbakri" tea in the train. it is eliachi flavoured tea and nothing else.

Rajasthan tour 21.12.17

woke up int the morning. train is running late by 30 mims. next station is Goa. the train Has pantry car but no tea so far. Aheli is sleeping in side upper berth. Rinku And Rik are sitting in  side lower. i was invesigating about stocks. i Want to enter stocks market in full fledged manner.

After quite a bit of permutations and combinations we managed to arrange seats close to each other. So far so good. Jai ma Kali.

reached howrah stn by 9.30 pm. yrain is scheduled to come at 11.40pm. so 2 hrs to go.
ticket confirmed but all are at different places.

Is it angina

1 .. Some times I feel a pain while sitting or just getting up from sitting position or mild walking. Question is "is it angina".
a)It doesn't have the same characteristics of previous angina.
b) no radiation.
c) on prolonged walking no pain.
d) always pain decreases on rubbing the chest wall.
e) on climbing 5th floor, 6th floor I donot have pain, or slight pain at lower part of chest which occurs more while taking rest than actual walking. But pain never ever occurs in upper left parasternal area.
f) my lipid profile is awesome.
g) my body wt is touching all time low. (65kg)
Today I climbed 6th floor thrice in quick succession, I had a mild pain  at lower part of ches at the end of 2nd round.

Rajasthan trip 20.12.2017

As always i woke up early. everybody sleeping. My Train ticket is still not confirmed. according to trainman 86 percent chance of confirmation is there.  SK Mondal also applied for VIP quota. Let's hope it gets confirmed.
  Hotels have been arranged but Dibyendu is not responding properly. I will talk to him today.

Rajastha tour

Rajasthan tour begins. We boarded ma Tara express to reach Kolkata. Tomorrow we will board Howrah Jodhpur express at 11pm.

Sunday

Yesterday I enjoyed a total holiday. No chamber ,no operation ,no journey to Kolkata. Absolutely nothing. I really cannot remember the last time I enjoyed such Hassel free holiday.
Anyway I shall be going to Rajasthan for conference. So  I shall be terribly busy in next few days.  As usual I shall be updating everything in my blog.

Islampur sd hospital

After 14 yrs I came to Islampur on a summon. I went to hospital . Lot of things changed. Binoy bera and Nikhil Bhakta are two people of my time  are still there. And  Actually I took hospitality of Dr B Bera. Met Bhakta in a Nursing Home. Met Bridges Babu of drugs India. He took a little time to recognise me but ultimately recognised. Also met Chandra Sekhar. He became little fatty. Everybody said I have lost a lot of weight.

today i boarded  sahid express at bolpur. Immediately a passenger whom i couldnot recognise stood up from his seat and offered it to me. i refused . but he insisted.  so i had to sit. probably i have done something good to someone.

Practice reaching the bottom. Barely reached double figure today.

Down hill practice

Again with the begining of winter, the harvesting season began and practice took a downhill turn. I cannot remember the last time I didn't have any operation on Saturday.
Today I donot have any operation on Saturday. No of patients in chamber  patients also decreased.
I hope it is universal. Last Wednesday opd was also finished quickly.
Review of blog revealed less no of patients in this period last year also. It will continue till March.

very hard to cope up

sometimes i feel like being milked like a cow. whoever gets chance squeezes me. so much expenses every day.  even i try my best i cannot cope up. everyday it is increasing.  even with a cardiac disease i try as much as possible. But unfortunately that is not enough. i am becoming irritable day by day.   i know everybody is having a not-so-good impression about me. But I always tried to be a good son, good brother, a good father or a good husband. but whoever meets me always wants this thing or that thing, either in cash or kind. Their hands are always up and never down. Added to this is the stress of my professsion.
Everything tightening the noose around my neck. i am unable to handle the stress. It is like riding a tiger's back. i donot know how i got there, but i just cannot get down.

7 days ago i operated for appendicitis. on opening abd i couldnot find appendix lot of adhesion between smallgut and uterus. i abandoned the procedure. pt was alright for 7 days . then she developed crampy pain abd. what are the possibilties

report about narayana is totatally frustrating. but i took up this as a challange. i shall leave no stone unturned to make my son successful.

Higher secondary results of narayana are below par. Aggrieved parents created a rokus in the school. Rik is very depressed. I am worried. Was it an incorrect decision to admit him in narayana. I advised him to keep cool and watch.

Dropped rik at hostel

As the summer vacation was over, we dropped rik at lahat hostel. He seems to be a bit relaxed now. Previously he was very depressed.

Shouted at sambhu

Today I shouted at sambhu. He misbehaved a bit. I felt Terri ly annoyed and shouted at him. Probably i should not have done that.

It was a moderately built female.patient  with good gall bladder. But instruments were not working properly. As a result i  had to toil for two hrs to get the job finished.

1st male patient. Easy gall bladder. Little bleeding from liver bed during the end. Otherwise okay.

Rik's result is going to be published today. I am very anxious. Was my father anxious like this when my madhyamik result was out. He never did well in exams. So logically he will not be able to do something extraordinary. But Father's mind doesn't go by logic. Oh god bless him. He needs your blessing so much.

Corrugated rubber drain

It drains too much. But following factors to be kept in mind.
1 it should decreasing. That is amount in first 4 hrs should be more than that in next 4 hrs.
2. Other parameters like pulse, bp, urine output, pallor and abd. softness should be okay.
3. Drainage generally continues for a day (24 hrs). Slight blood tinged should be okay.

Operation on a medical student

I did an appendectomy of a medical.student of BMCH.  The other day Sushovan, the gynecologist was operating on an incissional hernia. After toiling for more tha  an hour he couldnot proceed in that case. He called me and I did in 10 mins. So little bit of acknowledgement from everywhere.

আজকে অন্য একজন surgeon এর কথা আলোচনা করছিল। তার নিন্দা করছিল। তখন অশোক চ্যাটার্জী বলল"সবাই কি পুলক দত্ত! আস্তে আস্তে সবার উন্নতি হবে"। শুনে ভালো লাগলো।এই  appreciation  টাই তো সবাই চায়।

Alok death.

Alok hardly 35 yrs old died of heart attack. It actually left me in shock. Iam feeling  uneasiness, in chest etc. I know it is psychological, but it will Take some time to recover. Any news of heart disease makes me nervous. Now one 35 yr old man dying of heart attack has shattered my confidence.

Lean thin pt. Entry was easy. But cystic duct was very short. In fact I was dissecting the cbd as cystic duct. Later on i realised that it was cbd. Hopefully no bdi occurred. Rest was un eventful.

After 8 days stay in kol flat , i shall return home tomorrow. I booked ticket. After a long time rik will not be with us. 15 years. Long time.Aheli  is there otherwise it would have been a terrible experience.

Going to bardhaman from kol to do duty. new experience. Will it be a regular experience later in my life. Donot know. Many things happen in my life which I have never thought of in my wildest of dreams.

I spent my first night in kol flat. It was absolutely messy. Ate food in a nearby hotel. There is no khat. So we lay down on the floor at night on a bed sheet. But still we enjoyed it.

Saturday unit

I have been posted for 1 month in Saturday unit. On the first day no other faculty including rmo was present. I was alone with pgts. Two operations were scheduled. One open GB, another hydrocele.  I took part In. GB . It was done in quick time. Then I left. Took shantiniketan express. Came to Bolpur.  No other train. waited for sahid which was late by 1 hr.

She got admitted in class 1. She was scared to go. After coming from school she said she could not understand what the teacher said.

Anxiety

When I have anxiety and tension, i feel chest pain and start feeling unwell. How to get rid of this. This pain is not like angina. But it still causes depression in me. Last evening I ran upto 4th floor of my flat without any chest discomfort. So I believe it is not cardiac problem.  Yesterday PR Datta got blood transfusion. He said his disease has spread a lot. He was on chemotherapy. If he can live with cancer why can't I with a cardiac disease. When all the literature says there should be no problem at least for twenty yrs. My bp, lipid profile are all okay. No pain on exertion. I am on oil free diet. Why should I worry. Anxiety is very bad thing. I have seen my patients complaining of so many things out of anxiety.

Wake Up Call

Without getting depressed about the bypass opn i think I should take this as a wakeup call. Now that I am leading a healthy lifestyle,  it should prevent further spread of the disease if not reverse it. My motto should be to eat less do more yoga, lot of walking, decrease stress by taking less stressful cases and try to be healthy. Today I climbed to the station flyover without any chest discomfort. I should try to use stairs more frequently, and avoid lift. This is another way of burning my fat. It's a challenge for me and I must win it. When I will think of deviating from the lifestyle, i will think about rinku, rik and aheli. I think this will put me back to track.

Tomorrow I shall go to kolkata to see my flat. It's interior is complete. Now I have to purchase a khat, a tv ,tata sky, make arrangement for lpg. 

Feeling Unwell

Today I am not feeling well. Only a few days ago iwas feeling so well that even i was taking rich food.  When I feel unwell   i feel empty and vacant. I feel like dying. But I do not have chest pain. 
 I can walk ,run etc. Stress is killing me.  I am doing everything right. But I have to overcome stress
. I will not take stressful cases.

Knee Pain

It is still continuing. I do not know whether I will be relieved any time soon.

Sliding hernia

1. Gut was pushed back......Which is th treatment
2. No evidence of gut injury during the operation.
3 abdominal distension occurred immediately.
4. Pt has no discomfort.
5. No tachycardia.
7. Tongue moist.
8. No problem apart from abd distension.
9. I think it will be alright as soon as the ps comes.
10.I think things will be alright tomorrow.

Burger and momo

I am in bardhaman today. In the evening I took burger and momo. I donot want to take it but I couldnot resist it. I have to find a way out to resist the temptation. Now I shall mark in calender when I will eat something I should not eat. This may help.

Knee pain

Knee pain continuing, i got usg done. A subperiosteal collection was found at the lower end of femur..  It could be blood or pus. Terror struck, i did mri rt knee. It was blood. pain has decreased a lot. But still persisting.

Bile leak lap chole

My last lap chole in bmch had a bile leak for a few days. It was uneventful operation but still had a bile leak. Few days ago asim was telling that he had post op bile leak  in lap chole. So it is fairly common. I think it is due to inappropriate approximation of walls of bile duct by clips. So bottom line is if it is 100/200 ml. Just wait and watch. Things are going tobe alright.

Pain in the knee

New addition to my minor ailments is pain in the rt knee. Is it osteoarthritis. It is troubling me a lot. No pain at rest but cannot flex the knee jt, it is painful. It i persisting for 3 days.

Medium built female. Entry easy. Calor's triangle okay. Opened the gb during dissection from liver. Stone spillage occurred. Little messy near the end.

Lean patch

Lean patch in chamber is still continuing. I am really at a loss. What's happening. Ot patient's are not coming. Few ot patients left. My practice decreased or what is the problem.? I am craving for ot patient. It is really very pathetic. My skill has increased but no of ot patient is decreasing. What a paradox. I hope it is temporary.

Sweater টুপি থেকে এবছরের মতো মুক্তি

বাইরে এখনো ঠান্ডা আছে।

ঘরের মধ্যে আর ঠান্ডা লাগছে না। মাঝে মধ্যে fan চালাতে ইচ্ছে হচ্ছে। কিন্তু ভোরের ট্রেনে এখনো ঠান্ডা আছে।

Rinku's headache

I am too much concerned about rinku's headache. Now I shall try to find out and cure it.  I am also a bit tensed. She is also having some toothache. Is it migraine. Brain tumour is generally associated with some other feature like numbness or other neurological symptoms.

Appendix

Yesterday I was not able to do the appendectomy. After opening abdomen appendix could not be found. It was palpated at a distance wth total adhesion surrounding it. It could not be manipulated into operating field. So I had to stop saying that it has formed a lump. I think appendectomy would have been possible if abdomen was opened through a midline incision. It left me terribly disappointed. After 20 yrs of surgery i am not able to do this appendectomy. But I always think "patient's safety first." Heroism can kill the patient.

Running

Today I ran about quarter a km to catch a train. I was out of breath, bit no chest pain. I probably didn't run this much after the operation. So got relieved a bit.

Heat at opd rampurhat

Today I felt the heat at opd of rampurhat. I reached there at 11 0 'clock. Some people went there to super to complain against me. Even a person was fighting with me regarding non availability of medicines. This is the reason I left rampurhat . This thing gets repeated, i might have to leave rampurhat again.

10 yrs of blog

I was just reading old blogs. I found i started blogging on 4.2.2007. so it is about 10 yrs.  With cabg how long can I continue?

I did a lap.appendix today in bmch. This was my second lap appendix. Nicely done. Little.problem in orientation.

Private practice

No of patient visiting my chamber is going down. I don't know why. I did a few operation last few days.  Nh is full. I really do not know what is happening. Probably i did not face such a lean patch for last 2 yrs.  It is harvest season. But still only 6 patient in chamber and two operation in nh. is difficult to digest. My skill has improved a lot and I should be operating more. What is happening. Only a few days ago I used to  say that i won't see more than 20 pts, now I am craving for patients

We e went to joydev kenduli fair. During return we found a group of people was doing picnic. We stopped there. Found a place . Sat down on a mat. Took out our cooked food parota and chicken. Then took the food. That's how we celebrated picnic this year.

Private practice is at it's bottom. No of chamber pt is okay but ot pt is minimum. Is it temporary?  But nh is full. Others are having good ot pt.
