No more the same person

I am no more the same person. I used to eat up to the throat and digest eve I am having chest pain and upper abdomen pain. This is causing a lot of tension. God pls help me and make of healthy.

Targets for next year

1. Investigate my cardiac status
2. Take med and restrict life style
3. Reduce my weight by 5 kilo
4. increase my score by 50 L
5. Shift to the flat
6. Bring my parents to my flat.
7. Do advanced lap in rampurhat and bmch.
8. Try to build a house.
9. Half an hour to Rik's education
10 Investigate my cardiac status

Last year

1. Jayanta died.
2. Transferred to BMCH.
3. Started to do las in rampurhat and bmch

5. Bought another 3 cottah land roadside.
6. No tour.
7. chest pain.
8, flat is finally ready.

Chest pain

Near the end of Nov I started to have chest pain on exertion which is relieved on rest. Angina? I am little worried about it. My lipid profile is normal, I have to do echo and TMT to rule out the possibilities, what  is in store for me!

There is a chostochondritis component and a psychological aspect also. I need to investigate it. If life can be extended for even six months I can earn more than what I shall spend.

Rik

I said I shall look after Rik's education for half am hour everyday. But I couldn't. His exam has started. And lot of defficiency is evident. I must teach him otherwise he cannot make it to the top.

My lipid profile

Now or never

It's now or never. Ham gaining weight. Feeling uncomfortable. So weigt reduction is a must. From today I must take action. I shall review after 7days

Moving to flat

,,My flat is almost ready. Now I should seriously think about shifting. Let me consider pros. And cons. First the bads

Adaptation

<p>If I have to. Continue in bmch then I must enjoy train journey. I have no problem while going to Burdwan but while returning I donot get seat. And I have to stand in a crowded train for two hours. This is not easy. So I have decided to reserve seat while coming back. This will cost another 1000 bucks. I think I can afford it.

19th &7th L C

It was not difficult case but there was little bit bleeding. At the end of operation blood started to come in the drain causing few tense moment. After sometime blood stopped.

18th lap fail

My 18th lap was not successful. I Had to convert. It was in nh. 

17th lap chole

Therd was lot of adhesion. According to professor it was level 2 lap, I dissected the neck well, but there was bleeding in calot's triangle and it was cuterised. There was bleeding from liver bed. It was also managed .gallbladder was perforated with spillage of stones. In the end everything was okay.

Rainy day

It rained all night. When I woke up I did not feel like going to hospital but as I go only 3 days in a week I shrugged off my lethargy and moved. It is still very cloudy.

Monthly tickrt

My monthly ticket actually expired . And I remembered that when my train started to move. I had to jump from the running train renew the ticket. The train has left in the mean time. So I have to board a local train. I got a window seat. But it is going to overcrowded as time passes by.
I spent two weeks holiday at Kaitha. Now I am going to join today.

Purchase of land

I. Have purchased a 3 cottah land @ 4.5 lakh on roadside. This is my 3re land.
I have done first lap appendix operationat BMCH.

Rik

I am not able to spare enough time to him. Now I have decided to spend at least half am hour with his studies everyday. I shall review the situation every 7days. There is on point in blaming him he I can not perform my duties properly.

Lumber hernia.

incissional hernia is rare. Mainly it occurs after nephrectomy or pyelolithotomy. It may be diffuse without any fascial gap. It is due to muscle atrophy. Surgery is not advised in these cases. In cases where there is a muscle gap. Surgery has to be done. CT scan is diagnostic. Laparoscopy is the preferred way for surgery. Open techniques are difficult. The Peritoneum
has to be separated. from the muscles and a mesh has to placed peperitneally. Muscles are to be closed over the mesh. I have done one case today.

16th l/c

After quite some time lap has been started in hospital. It was nice easy. Entry was perfect. Total operation took about 40 mins.
I want a little more practice before speeding up.
Biometric system is going to be started soon. Little worried about it.

Mes review

As long I am in burdwan I think I should be reasonably happy. Only thing disturbing me is daily journey. While going to burdwan trains are not crowded and seats at available in plenty .but the problem occurs when coming back. No ac coach in Balurghat express and trains overcrowded. More often than not I have to stand all the way for 2 hrs. Now I having a pain in my pt knee it. This may be due to prolonged standing.
I have done 2nd prostatectomy. And I am feeling confident about prostate operations. Someday I shall do in Rampurhat.

Destiny and myself

I cannot control everything. Sometimes things happen in a way which was not intended. Inspite of my best effort thongs happen in other way, complications of operation occur. Those are few but enough to disturb night's sleep. How can I get rid of it? How can I do absolutely complication free operation. Is it possible. Probably not. I have decided that I shall exercise utmost skill and knowledge,and leave the rest to almighty. If He wants to glorify me nothing can stop, and if he has other thoughts for me, there is no stopping again. I shall do my best and hope for the best,

5th and 15th L/C

Again a lean and thin patient was chosen. For the first time entry was perFect and was achieved at the first shot.
Little bit of adhesion here and there. There was a little bit of bleeding also during operation and it was managed. It took about 2 hours to complete the operation.
I am taking a little more time than standard. But I cannot help... This is learning curve.

Puja

Puja season begins with Biswakaarma puja, it is going to end with Saraswati puja in Feb

Dipak is not anesthesist after my heart. He licks the feet oftother surgeon.Today my cases were scheduled at 7am, they did not turn up even at 8 am,
I cancelled it and came back.

4th and 14th LC

My 4th lap chole was a medium built pt. Access to abd has become constant problem which I cannot overcome. Cystic duct was short. Artery was a little away. It took a little time but everything was shorted at the end.

my first two lap came in the f-up. They are alright.

Prostate and biometric system

Today is the perfect blend of good and bad. I did one acute gallbladder operation and anal fissure operation in Bengal nh. Then I went to sign only to see the biometric system has been installed. It is still not operational. Then I operated on a BHP patient. This is one operation I always wanted to do. I did it finally. I am happy and sorry both at the same Time.

3rd lap chole nh

it was medium built pt. Entry was again difficult. Afterwards it was easy. Lmg cystic duct and artery was clipped togather. Every thing was okay except gb was punctured near the end by the hook.
My last two pt was okay and discharged.

12th and 2nd lap

My 2nd lap in nh was a lean and thin pt. Like the prev pt entry was a little problem, there was no adhesion. But anatomy was a littlle confusing. Cystic duct was short. Aptery was not found. So I had to dissect patiently. At last artery was found and clipped. The whole operative procedure took mre than 2 hrs, I think it was a good effort. Anaesthesist was very unhappy,

Lap chole 11and first

Dr A Chatterjee bought lap instrument. Today I have done my first lap chole in NH. I also did appendectomy using the same ports. It was neatly done. Everybody also appreciated it and demanded treat. Keshri was very happy. My parents were also happy. Name of the patient was Tuktuki Bibi, of Kaitha. It was as if my dream came true. I am very happy today.

Last week

Last there was Eid of Muslim and 15th August. So there was a lot of holidays. It became very stormy. At least five families including Gargi
and Utpal came to my house. They all had some problem. Finally everything seems to be in place.
Few days ago I operated on a patient.It was a huge tumor involving face. It was successfully removed. It was published in a local newspaper and shown in a local channel. My father was very happy about it.

Lap instrument

Dr chatterjee finally bought lap instrument. Now it is my turn to reciprocate.
I am searching for a case from my locality.

9th and 10th lap chole

<p>My experience with lap is getting bitter. It was a mess today. Cystic artery was torn. I was about to convert but somehow bleeding was controlled ny diathermy. I lost my patience, tried to remove the gall bladder quickly. Torn it with spillage of stones. They were left inside abdomen in large number. I was far from being satisfied. Next day I shall do more patiently. Dr A chattergee said he will buy lap instrument. Let me see.

Local train

Today I deliberately left express train and boarded the local train. Just train fare has not increased in last ten years, everyone can afford the express train. So there is more crowd in express train than in local train. I couldn't manage a window seat in express train so boarded the local train. Main draw back is that I am travelling with a patient party in the same coach.

Loose motion

Yesterday was Rik's 11th birthday. I purchased a remote car for him in the evening. As my son is growing older I am finding it difficult to find B'day gift for him. Rinku cooked the spicy food in her own style and as a result I am having loose motion. My tummy is no more the older one which could boast of digesting Anything. I have to be careful specially about foods in train. They are the the most unhealthy food of the world. Gone are the days olden and golden when I could digest food made up of iron.

Prostate operation

Actually I have assisted prof Biswas in another BHP case. I think I can gradually learn the art of Frayed prostatectomy It is a blind enucleation like fibroadenoma breast. I like Prof Biswas approach to surgery that you always learn. You may be housestaff or visiting surgeon---- you will always find something to learn.
Prostatectomy is one operation I always wanted to do. My joiningvote MES is fruitful.

July Practice

I was having a reasonably good practice in the month of July. Total income was 265000 including the salary. LET ME SEE it is maintained. I prayed to GOD For a correct blend of pp and mental peace. So far that is happening.

8th lap chole

8th lap chole was again a hefty patient. Dissection was alright. Clip applicator went out of order. Intracorporeal knot was given. It was not easy. End was messy. Gelfoam was given on lover bed.
Limnerg flap operations for pilonidal sinus was done.
Overall it was a good experience.

Mes revew

It is now more than two years since I have joined MES. During that time I was not sure whether it was a correct decision or not. Let me review.
First the bads
1. Transfer to Calcutta may spell disaster for me.
2. Sometimes change of unit disturbs the rhythm.
3. Leg pulling is there.
4. I have become a daily passenger.
Now the goods
1. I have become a more confident surgeon, and it has a positive impact on my practice.
2. Finally I am doing lap chole
3. No tension hospital related. Today is ad day and opd but I am totally relaxed. Which I couldn't think in my Rampurhat days.
4. I am learning new operation like microdochectomy. I am also learning new technique.
5. So far I have no regrets.
6. Few hospital doctors like P R DATTA, HIRANMOY, LOTIF, S GHOSH had to leave job justified my decision.

Travelingin a crowded train

Travelling. In a crowded train has become a routine for me. I have tried everything.  I tried to board the first coach behind the engine,the last coach, the for disabled, coaching the middle. Sometimes it works. I have calculation too. Like today is Saturday or sunday,so the crowd will be less. Almost always I am wrong and find myself standing at a corner of coach. Bottomline is there is less train than needed,

today I have seen six patients in Burdwan and operated on two ptients, I think that is great.

Microdochectomy

Today I saw this operation for the first time. It was done for bloody discharge per nipple. It was not done in a convincing way but still a new experience. That is way for I came to MES

Traveling in train

Now traveling in Tran daily four hours has become a habit. I don't get bored any more. With this I also decided to go on half fast on eetwos days. If that helps to reduce my weight. Let me see.

7th laparoscopy

IT WAS. An inflamed gb with lot of adhesion. Though I tried to do it classically. It was not to be. Somehow it was done. Anatomy was not. very clear. Stones spilled during gb extraction. IiAnyway gb can removed even in difficult cases laparoscopically. Next I did a fibroadenoma removal in Bengal N H.

Enjoy tension

While talking to Prof R Biswas about tension in surgeon's life he said you should learn to enjoy tension. This is an entirely new concept. Let me see if I am good at it. Gradually I am becoming a money laundering machine in my family.

6th lap chole

Today I performed another lap chole. There was lot of adhesion all around. I did it in less than an hour. clipped both cystic duct and artery with a single 400 clip. Gradually life is becoming easy.

Shyamsundar

Last Friday I went Shyamsundar and Raina for pp. There were four patients in shyamsundar and none in Raina. I am doing this because I want to develop a base in Bardhaman.

5th lap chole

Today I have done another lap chole. It was a.male patient. There were few positive and negatives. I had problem entering the abdomen. But dissecting gallbladder from liverbed was nice. Retrieval through the port was uneventful. I am gradually learning the art of laparoscopy.

Experience in new unit

Prof R Biswas was waiting for getting into some understanding. We had a talk in the opd and everything is in order. Day off on Tuesday and Saturday. Alternate thursday round. What else One can ask for. Pgts proposed to give ward round at 8 p.m. Prof Biswas is good in laparoscopy. But he is not so good in other surgeries probably.

New unit

From today I am posted in Wednesday unit. Dr Rabi Biswas is in this unit. He is professor. The are wanting me for quite some time. Yesterday Dr K majumdar HOD agreed.  Prof Biswas always talked about understanding. I donot know how he can do it. Hopefully things donot turn sour on the first day. God pls be with me.

A/C not working

Godrej AC is not able to cool the hot noons. Though it is making the night chill.

For the First time I have operated on a dozen patient in asha nh on sunday,

MCI Visit

One of the bad thing of MES is MCI visit . Today there will be MVI Visit in our hospital. So I have to go early. I am going by viswabharati . I have to take all the original document.

4th lap chole

4th patient was a classical gb patient. FAT FERTILE AND FORTY. I thought lap will not be possible. There was lot of adhesion. It took time but ultimately it was done. Though it was messy at the end. Gb got perforated, stones spilled out. They were only partially recovered. I need to improve. Especially during removing from liver.

Visiting surgeon

Now that I am visiting surgeon of a unit. I must behave accordingly. Though I like very much to be present in emergency OT and I love to emergency operation. Buy I must refrain myself from doing that.

Dr. Jayanta Biswas DCH, MD(MED)

Jayanta was my co house staff and Roommate at NHQ. He had a PCNL for renal stone at woodlands NH. He had severe post op bleeding leading to shock from which he could not recover. He passed away on 7/5/12. He was nicknamed larva for his short height. He finally matured and flew away.

DIRTY POLITICS

I became victim of dirty politics again. Dr K K Dey. who is known for his corrupted attitude. played spoilsport this time. He told HOD that he doesnot want me in his unit. I had to change my unit. I am angry. I know that I cannot avenge this. But people are actually falls in their own grave.

I have to accept the situation. May be God has done for my betterment.

3rd lap chole

I have done my 3ed lap chole. It was easy looking gallbladder. It around one hour. I plan to lap outside after doing 20 cases of lap in hospital. So i am counting.

2nd Lap Chole

I have done 2nd Lap chole. It was very difficult to begin with. Gallbladder was not visible. I went on dissecting and ultimately it was done. I am having some problem with Liverbed bleeding. I like to count up to 20 before starting it outside.

Lap chole

One of the reasons fojoining MES was to do Lap Chole. Last saturday I performed one lap chole skin to skin. name of the Patient was Rabeya.
I hope to do many more.

BMCH

I have joined BMCH. HOD asked me to work in his unit. There is another Asst. Prof in this Unit. Dr Sandip Ghosh. He is a nice Man. HOD Dr K  Majumder is a soft hearted person. I think I can do laparoscopy in this unit. Let me see.  My PP seems to take an upward turn. I have srarted doing cases in Asha NH. I want to do chamber in BMCH. I have asked MRs to look for a suitable chamber.

BMCH

Yes, Results came out on 18/1/2012. I am promoted and transferred to BMCH. Principal was absent on 19/1. So I was released on 20.1.

I joined BMCH on 21. 1. 12. I want to join Jyotida’s Unit. Let me see if that happens.

I have to go once to Bankura to bring my bag which I left in Shibananda Lodge. I have to bring my LPC and service book also.

I am happy that I donot have to work again with Dr S B saha and Dr Ashis Saha.
I couldnot meet Dr pulak Samanta Our M.O. – He was a nice guy.

WILL IT HAPPEN?

our result of promotion and transfer is going to published next wk. Shall I be promoted? shall I be transferred to Bsmch? WILL IT HAPPEN THE WAY I WANT IT?

Plans and expectation in 2012.

If the world does not end in 2012 I have my plans and expectations.

1. I expect to be promoted and transferred to BMCH

2. I must try to settle down in Burdwan and try to develop PP there.

3. I must shift to my flat.

4. My balance sheet  should read 1C at the end of 2012.

5. try to shift my family from  Rampurhat towards the end of 2012

Nothing right

Sometimes nothing happens the way we want it. I am going through this phase. My flat is not ready. There is a possibility of my promotion and transfer--- results are not being published. There is a warrant issued against my name for not attending summon. Kesri has started painting our flat causing a hell lot of problem. Nasim has entered NC Datta NH.  Nothing is going right. PP is taking a downward slide.  Best policy is to wait  and watch patiently.

2011 a look back

1. it started with death of Dr Kader

2. I had chickenpox

3. I appeared in exams but no promotion.

4. Aheli was born on 8/3.2011.

5. 34 year old left front govt was toppled.Ms Mamata Banerjee {TMC) became CM.

6. I bought a necklace for Aheli. It costed Around 1.5 lacs (58 GRAMS)

7. I arranged Annaprasan of Aheli in October.

8. My PP Took a downward turn towards the end.

9. We went to Kochi, Kerala In DECEMBER to attend conference.

10. I finished at about 70 L

11. Govt. Abolished KVP. So I have to Find Other means of regular Savings.

12. MY Flat is not Ready yet.

kochi ,the end

The other day we went to broadway road for marketting. Contrary to it’s name it is narrow but good for shopping. Spices are sold here. we bought spices for our home.

Then we went to wood handicraft shop near KTDC office at Summangam road. There are two shops . Lots of wood handicrafts are available at a reasonable price. Again one has to do a little bit of bargaining. I bought a chess{wooden) Few elephants, bowls etc. One should not buy these at Fort Kochi because they will cheat you. Finally we came back In SHM-TVC express, A/c 2 tier.

kochi day 6

Yesterday we went to conference. There was a topic on medical photography. I learnt a few things. Discussion breast ca confused a lot more.

In the evening,  We had a boating in backwater. after that we went to Broadway road near marine drive. I thought that is the best place for marketing. It was a narrow road with different shops on boath sides. People love to buy spice there.

Then at 7 PM We went to wellingdon Island for banquet dinner. This island was artificially built by British and was named after their viceroy. It was a really gala dinner. Lots of tiger prawn to eat, Actually i ate only tiger prawn and ice cream. I never ate so much of prawn.
